Each Student must have on file a completed student health information form, available for download here.

Any adult travelling overnight with our students must submit to a criminal background check. Potential chaperones must complete the LCS Criminal History Search Consent Form and return it at least one week prior to any trip, with a copy of the adult's drivers license.  No adult may travel overnight with our students without completing this background check.

Chaperones are essential for the safe transport of our students when we travel and even on Friday nights in our stadium at home.  For the safe and efficient operation of the band, all chaperones must adhere to the guidelines posted here.
